The Business Problem
The Christ Hospital Health Network needed to make it easy for patients, families, and staff to nominate caregivers for two signature awards: the Daisy Award for nurses and the Tulip Award for patient care associates. They wanted clear, inviting printed forms with space to write a nomination or scan a QR code to complete it online. Beyond encouraging nominations, they also needed large, durable banners to display winners’ names, updated each year as new awardees were added. They needed a partner who could deliver high-quality, effective print materials that were both practical and professional.

These 8.5×11-inch trifold forms describe the award and have space to collect the nominator’s contact information. The back has room for writing the nomination itself. The form can be folded, sealed, and mailed or people can use a QR code for easy online submissions. The design made it simple for anyone to nominate a deserving caregiver.
