Challenge: The Pharmacy Department at St. Elizabeth needed a book for their new patients to welcome them to the healthcare community, answer frequently asked questions, and share new patient forms to be completed.

Our team collated the provided materials so that relevant information paired with the required new patient response forms. These forms are easily detached to be submitted to St. Elizabeth with the included envelope. Given the envelope’s large size, our team formatted, folded, and inserted into the pocket folder for convenient and more consistent completion of forms. These combined solutions created a user-friendly, easy-to-understand book that prepares new patients for their next appointment.
